Transitional Aged Care Program

Royal Rehab provides the therapy component of the Transitional Aged Care Program to clients residing in the Northern Sydney Local Health District (NSLHD).

 

Royal Rehab works in partnership with NSLHD to administer the program in both a client’s home (TRANSPAC NS) and at a residential setting in Belrose (Northern Sydney Transition Care Unit).

The program provides time-limited, multidisciplinary therapy, nursing support and care services to persons aged 65 years and older to enable their return home from hospital following injury or illness.  Royal Rehab provides a range of therapies from specialist allied health professionals which include occupational therapists, physiotherapists, speech pathologists, social workers, dieticians and allied health assistants.  We aim to support people to continue to live in their own homes and participate in their local communities, independently as possible.

Referrals to the program require an assessment by the Local Aged Care Assessment Team.  To be eligible, a person must be referred directly from their inpatient hospital admission.
